Latest Original bank statement for the last 6 months (stamped and signed by the bank)
Salary Certificate / slip for the last 3 months AND
If the applicant is employed, employment contract and NOC from Employer on approval for holiday
If the applicant is a company owner or self employed, Certificate of registration of the company
If the applicant is Sponsored, Official Invitation ( Zaproszenie) registered in Urzad Wojewodzki by the sponsor in Poland.
If the applicant is retired, Pendion statement for the last 3 months
***Photo requirements
1) Size: 3.5 x 4.5 cm
2) Photo should be in colour, sharp and with appropriate contrast
3) Printed on high quality paper with light background.
4) Not older than 6 months
5) Facing the camera, showing clearly the eyes and both sides of the face from the top of the head to the upper part of the shoulders. The face should occupy 70-80% of the photograph area.
6) No headgear, unless if you are wearing of religious nature and the photographed person should look ahead with his/her lips closed.
7) Hair must not cover the face.
In case of person wearing glasses
1) No dark lenses
2) The frame must not cover any part of the eye
3) There should be no light reflexes.

Invitation from the inviting company / organisation.
FAMILY VISA
In addition to the above mentioned points a special invitation (“ZAPROSZENIE” in Polish language) registered in Urzad Wojewodzki by the sponsor in Poland. covering the whole period of the planned trip has to be submitted together with the visa application form & Employment information with relevant documents and the details of itinerary. if the host is not a permanent resident in Poland- Personal Invitation specifying accomodation details and financial arrangements. Certificate proving the existence of family relations, if applicant is visiting relatives.
SPECIAL NOTES
1) For Minors travelling without either of the parents,
a) NOC on Rs.100/- stamp paper duly notarised by the home department & with parents signature is a must.
b) Written consent of both the parents / Guardian having the care and custody of the applicant.
c) Copies of the passport of the parents. If not applicable, birth certificate of the applicant and copies of the ID cards of the parents
2) Pax CANNOT go directly for the interview unless the consulate wants them to come personally, which will be decided after the application is made. That means that even Blank passport can be applied in drop box. The decision taken by the consulate after that will be final.
